Welcome to MyChart

Your healthcare information – anywhere, anytime

As part of providing world-class health care, Northwestern Medicine is transitioning to MyChart, a secure, confidential patient portal that allows you to access your medical information and send non-emergency messages to physicians and healthcare providers of Kishwaukee Physician Group, Northwestern Medicine Regional Medical Group and Northwestern Medical Group.

Beginning April 30, 2017, you can manage and receive information about your visits, tests and labs through the MyChart online portal. MyChart is convenient and easy to use from your smartphone, tablet or laptop computer.

With MyChart, you can:

  • Access your list of allergies, current medications, current health issues and immunizations.
  • View test results.
  • View discharge summary and instructions from inpatient stays and emergency room visits.
  • View the health record of family members through Proxy Access.
  • View and pay bills.

If your primary care physician and specialist(s) are part of Northwestern Medicine Regional Medical Group, you may use MyChart to:

  • Request medication refills.
  • Send non-urgent messages to the Northwestern Medicine Regional Medical Group's medical team.

If you were previously enrolled in MyKishHealth and NextGen patient portals, you will be able to access and read information from visits, tests and labs that occurred before April 29, 2017. No new information will be entered into these legacy portals after this date.

In order to use MyChart, you will need to enroll by visiting nm.org/MyChart/Kish, starting April 30, 2017. There is no cost and it takes only a few minutes to get started. You will need to create new usernames and passwords for MyChart.

Other frequently asked questions about MyChart

How do I enroll in MyChart?

Getting started is easy:

  1. You will receive a personalized access code from your healthcare provider or you can request one online at nm.org/MyChart/Kish. It will be printed on the “After Visit” summary. The code stays the same for 90 days. If it is not used, a new code will be generated every 90 days and will continue to be available for you to enroll.
  2. Visit nm.org/MyChart/Kish and select the “Activate Your Account” button.
  3. Follow the “Sign Up” instructions, using your personalized access code.
  4. Create a username and password. Be sure to remember this information to access MyChart account in the future.

After these steps are completed, you can login to your new MyChart account with your username and password.

Can a patient view a family member's health record in MyChart?

Yes, Proxy Access allows a person to log into their personal MyChart account, and then connect to information regarding another person. A patient over the age of 18 can appoint any other adult as a proxy. For patients under age 18, proxy access can only be given to their parents or legal guardians. To request proxy access, patients must fill out a proxy form available on nm.org/MyChart/Kish.

Can a patient ask questions regarding a family member from my MyChart account?

No. Communication within MyChart is part of each patient’s health record and communication must be for the specific patient. This is to ensure patient privacy and safety.

MyKishHealth Portals

If you are currently using online patient portals through KishHealth System: you can continue to use MyKishHealth and NextGen patient portals to view information from visits, tests and labs that occurred before April 29, 2017. In order to use the new MyChart patient portal, you will need to enroll in MyChart.


Kishwaukee or Valley West Hospital MyKishHealth Patient Portal

This interactive web portal empowers all KishHealth System patients and their families to take a more active role in their care by providing easy, secure access to health information and online communication with KishHealth System staff. All of the information in MyKishHealth comes from your KishHealth System Electronic Health Record. This ensures that you have access to the most accurate, up-to-date information possible.
